Founder/Executive Director, Physical Therapist

Sophia Patel, DPT
Sophia’s passion for physical therapy began as a teenager in Malawi, where she volunteered at a nonprofit providing rehabilitation services for individuals with special needs. That early experience ignited a lifelong commitment to helping others build functional independence and joy through movement.
Now with over 22 years of experience as a physical therapist, Sophia has devoted her career to working with children with complex rehabilitation needs. She earned her Master of Physical Therapy from Missouri State University and her Doctorate of Physical Therapy from the University of South Florida. She is a board-certified pediatric clinical specialist through the American Board of Physical Therapy Specialties (ABPTS) and is certified in Neuro-Developmental Treatment (NDT) through the Neuro-Developmental Treatment Association (NDTA). Sophia has received advanced training in a variety of therapeutic approaches and her integrative approach draws from these modalities to create customized, effective, and meaningful therapy experiences for every child she serves.
After making Eugene her home, Sophia fell in love with Oregon’s natural beauty and its people. While working in a community-based pediatric program, she saw firsthand the gap in services for children with significant needs and the often fragmented nature of care. In response, she took a leap and founded Functional Foundations Pediatric Services.
Sophia believes in a strengths-based, family-centered approach that brings fun, purpose, and empowerment to every therapy session. She is committed to building a collaborative team of therapists and providing individualized care that fits the unique needs of each child and family.​
Office manager

Nina MacKenzie
Nina has over 20 years of experience in the medical administrative and insurance industry. As our Office Manager, she brings both professional dedication and personal passion to serving children with complex medical needs and their families. Inspired by her own journey as a parent to a child with a brain injury, she finds purpose in supporting families who often feel unseen and overwhelmed. Her work helps transform grief into advocacy — ensuring that every child, regardless of ability, gets the care, dignity, and opportunity they deserve. Outside the office she recharges near the water, unwinds with a good round of golf, loves Oregon Duck football, and finds joy in raising her own two kids — one a budding singer, actor and golfer, and the other a very brave, funny, opinionated little warrior navigating life with a disability.
Physical Therapist​

Leah Johnston, DPT
Leah grew up in Tennessee and lived in multiple states after graduating high school. She earned Bachelor of Arts degrees in Dance and Psychology from Goucher College in Maryland in 2005. After that she spent time working as a raft guide, teaching Pilates and performing with several small dance companies in San Francisco. She moved to Oregon in 2011 to attend Pacific University where she received her Doctorate in
Physical Therapy.
Leah initially chose physical therapy as a career because it combined her interests of exercise, medicine, and engaging patients in the healing process. Her career in Physical Therapy has been in the areas of orthopedics, pelvic health, and geriatrics for 9 years before joining the team at Functional Foundations. She is excited to be starting a new chapter in her career with a focus on pediatrics. Leah has always enjoyedworking with children and strives to provide care that will increase functional mobility and improve children’s ability to engage in their community. She is thankful to be part of a supportive team that is helping her continue to improve her skills to best serve the children that come to Functional Foundations for their care.
When not at work you can find Leah hiking, trail running, kayaking, skiing, or on an adventure with her husband, son, and dog.

Pamela Versage, DPT
Though officially retired, Pamela continues to share her time and passion at Functional Foundations Pediatric Therapy, where she brings over 40 years of experience working with children with complex medical needs. Her deep knowledge and unwavering dedication make her an invaluable part of our team.
Pamela’s presence in the clinic is a true gift—her mentorship supports the growth of our clinicians, and her enthusiasm for pediatric care reignites our collective passion for the work we do. She embodies the spirit of lifelong learning and service, always ready to lend insight, guidance, and encouragement.

Physical Therapist

Anna Cardosa Gray, PT
Anna earned her Bachelor of Science in General Science with a focus in Pre-Physical Therapy from Oregon State University, and went on to complete her Master of Physical Therapy at Idaho State University in 2003. She spent over two decades in Arizona, working with children across a variety of settings, including home health, early intervention, outpatient clinics, and both public and private schools.
Her experience includes supporting children with multiple disabilities, intellectual disabilities, autism spectrum disorder, gait challenges, visual impairments, and dual sensory impairments. She brings a compassionate, collaborative approach to care and is passionate about helping each child build strength, confidence, and new skills.
Now back in Oregon and living on her husband’s family tree farm, she’s excited to continue her work with children and families in the Pacific Northwest. Outside of the clinic, she enjoys reading, quilting, swimming, Pilates, watching Star Trek, and exploring the beauty of Oregon.
Physical Therapist

Danika Anaya, DPT, PCS
Danika graduated from Purdue University, then went on to earn her doctorate in physical therapy at Pacific University in Oregon. She has over 18 years of experience working with infants and children in Washington, Kentucky, and Oregon. Danika has worked in hospital settings and outpatient neurodevelopmental clinics. She continues to provide care in a NICU, in addition to serving our patients.
Danika has completed advanced certifications, including Pediatric Clinical Specialist, Certified Neonatal Therapist, and Certified Neonatal Touch & Massage Therapist. She is formally trained in the General Movement Assessment of infants, the Test of Infant Motor Performance, and manual edema mobilization.
Danika has a passion for learning and growing in knowledge to better serve each child and family in her care.

Mina Stollberg, OTD, OTR/L
Mina is from Buffalo, New York. She attended Duquesne University in Pittsburgh, Pennsylvania where she earned her Bachelor of Science in Health Sciences, and then her Doctorate in Occupational Therapy in 2023. She moved to Eugene shortly after in 2024 to join her boyfriend, who is a PhD student at the University of Oregon.
Mina has clinical experience working as an OT in school-based pediatrics in both New York and Oregon. She has known that she wants to work with children with disabilities her whole life, growing up alongside her younger brother who has autism. Although an emerging practitioner, Mina is excited to develop her skills at Functional Foundations and learn from other experienced therapists on staff. She has years of volunteer and work experience working with children and young adults with various conditions, which she knows will support her in this new position. Mina is excited to provide the individualized child and family-centered care that Functional Foundations strives to deliver. When not at the clinic, you can find Mina exploring the beautiful outdoors of the Pacific Northwest, trying out a new recipe in the kitchen, or cheering on her favorite football team, the Buffalo Bills!
